What T2D™ Offers

T2D™ connects people with real medical needs to cost-effective versions of GLP-1 medications through their membership program. Their mission includes a commitment to high-quality service—and when follow-up medical care is needed, Navigating Wellness is here to support you.

Navigating Wellness Services for T2D Members

As a T2D member, you can access discounted virtual care through Navigating Wellness:

Visit Cost: $75 per telehealth visit
Visit Frequency: One visit required every 3 months
Between Visits: Access to secure provider messaging via our CharmEHR texting portal for acute needs

Sign up for a T2D™ Membership at T2D.com
Once your membership is active, visit NavigatingWellnessPrimaryCare.com or NavigatingWellness.clinic
Click on Appointments/Links
Select T2D Provider
Choose a date/time or join the waitlist—we’ll do our best to match you
Complete intake forms and required documentation
Upload your preferred payment method
Upload recent lab work (within the last 6 months), including:
- Complete Metabolic Panel (CMP)
- Hemoglobin A1C
- Lipid Panel
- Thyroid panel (TSH minimum)

Don’t have recent labs?
We can order them to your local lab, often covered by insurance

Or, self-pay pricing is available: $50 at Quest Diagnostics (current as of May 2025)

Note: New York residents will have different pricing—please inquire

Eligibility Requirements

All patients must meet appropriate clinical criteria for GLP-1 therapy based on their health history, lab results, and current medications. Our providers will review this during your virtual intake appointment.
